APPENDIX 2
Informed consent form for participation in
Open Night Nights.
The purpose of this
document is to make explicit the nature of the proposed involvement between the
researcher (Samantha Webber) and the person agreeing to supply information (the
participants) and to record that the research participants understand and are
satisfied with the proposed arrangements. The researcher leading this research
is Samantha Webber, her contact

The project Open Mic Night : The purpose of the project is to carry out inquires, in which I have
chosen, to carry out an injury at Open Mic Nights. This is being done so that I
am able to gain as much information on the outcome of these invents in the
professional community. This information can be gained in a variety of ways,
but the tool I chose to implement is the one on one interviews.
What participation in the study will involve: I will ask you three to four questions and
it will take no longer than 10-15mins I will be recording the interview and
writing notes.
Use of data: The aim will be
eventually to present the research along with data collected in other parts of
the study, in appropriate contexts, academic form for a BA final research
project. The researcher, Samantha Webber, will not name participants but
identify using pseudonyms. The
participants will have access to copies of any presentations based on the
research.
Anonymity of participants: All information acquired will be treated as confidential. References in
publication, talks etc to particular organizations, individuals etc will be
anonymous and features which might make identification easy will be removed.
De-brief and feedback: A date will be set in May 2012 (or before) for de-brief, feedback and
an opportunity for participants to see the final findings of the research.
Samantha will contact the participant closer to the time of this date.
